Every institute having Xray equipment being used for various purpose, these institute need to register and get the license to operate for each equipment with AERB (Atomic Energy Regulatory Board).

To get the license, each institute need to register with AERB on eLoara and apply for the institute registration and follow the process of equipment procurement till licenses. CT Scan and Cath Lab (Interventional Radiology) required one Radiation Professional as RSO, then only institute can get the license for operation.

Documents required for institute registration are as follow:

A. Owner/Director Details:

2. Proof of Employer ship/Declaration (Attached herewith)
3. Mobile no & Email (Attached herewith)

6. Address with PIN.

B. Institute Details
1. Name of institute:
2. Address with PIN.
3. Landline no
4. Email id

5. Light Bill of the place

6. Index II (If owned)/ Leased document (if rented)

7. PAN Card of the institute if Company/LLP/Partnership.

8. Partnership Deed/LLP/Pvt Company formation Certificate

(Note: Gov registered any document needed as registered Name & address proof)

The following Two documents are required to be filled with details, printed on institute letter head, signed and stamped. (these are given in word format to make it easy to fill the details)
